Just found another - the Repose and Tyranny Cleric domains have the same description text apart from the name referral.
Fixed for 1.32.

If a cleric takes both the cold domain and the water domain, the spell ice storm appears three times, two separate entries as a fifth level spell and once as a fourth level spell which, when selected, fills a fifth level spell slot. domains.2da only includes spell id 368 (ice storm) twice, as a fifth level cold spell and as a fourth level water spell. Where the second fifth level spell is coming from is beyond me.

I'll have to report this to Obsidian as there isn't anything I can do about this bug. Since domain spells are not really "normal" spells known the engine does some chicanery to add it to the spell list. Had you picked a second domain that did not have ice storm you would have it show up correctly (as either a level 4 or 5 spell). This is a problem even with stock domains as Trickery and Chaos share Confusion and have the same problem. On a similar note, Water and Plant both grant Poison so it shows up twice in the spellbook (though at the same level).
Domains are… flaky.
